The effect of psychiatric comorbidity on pregnancy outcome among SARS-CoV-2 positive women with asymptomatic and mildly symptomatic infections remains largely unknown. We reviewed the electronic medical records of all pregnant women who received care at Mayo Health System and tested positive for SARS-CoV-2 (RT-PCR) from March 2020 through October 2021. Among 789 patients, 34.2% (n = 270) had psychiatric comorbidity. Of those with psychiatric comrobidity, 62.2% (n = 168) had depression prior to pregnancy, and 5.2% (n = 14) reported new-onset depression during pregnancy. Before pregnancy, 65.6% (n = 177) had anxiety, and 4.4% (n = 12) developed anxiety during pregnancy Thirteen percent of SARS-CoV-2 positive pregnant women (n = 108) received psychotropic medication during pregnancy. In addition, 6.7% (n = 18) and 10.7% (n = 29) of pregnant women with psychiatric comorbidity had documented nicotine, cannabis and/ or illicit substance use during and prior to pregnancy, respectively. We depicted a significantly higher risk for cesarean delivery [35.6% vs. 24.9%) in asymptomatic and mildly symptomatic SARS-CoV-2 positive pregnant women with psychiatric comorbidity. In conclusion, the prevalence rates of depression, anxiety, and prescribed antidepressant medications during pregnancy among asymptomatic and mildly symptomatic SARS-CoV-2 infected women were substantially higher than average, which negatively impacted pregnancy and neonatal outcomes.

Introducción: La COVID-19 fue declarada por la Organización Mundial de la Salud como pandemia el 11 de marzo de 2020 por su complejidad y rápida expansión mundial. El municipio de Sandino presentó su primer caso positivo el 6 de octubre de 2020. Objetivo: Caracterizar el comportamiento clínico-epidemiológico de la COVID-19 en Sandino, en el año 2020. Métodos: Se realizó una investigación descriptiva y transversal, del 11 de marzo al 31 de diciembre. Con la información obtenida de historias clínico-epidemiológicas, resultados virológicos, pesquisa domiciliaria y laboral, atenciones por infecciones respiratorias agudas en consultorios y policlínicos, y los partes diarios de la Dirección Municipal de Salud, se realizó un análisis estadístico cuya información fue depositada en base de datos en Excel. Resultados: La tasa de incidencia fue de 528,23 por 100 000 habitantes; sin fallecidos. La mayoría de los casos ocurrieron en la ciudad de Sandino. Hubo mayor frecuencia de afectados, entre 41 y 60 años de edad, y el sexo masculino, mientras el 55,73 por ciento, fueron asintomáticos al diagnóstico. Los síntomas más frecuentes fueron la tos, la fiebre y la congestión nasal. Se diagnosticaron entre uno y 13 casos por día durante los 63 días que hubo positividad. La transmisión familiar fue del 68,23 por ciento y la laboral del 50,52 por ciento de los casos. Conclusiones: La COVID-19 tuvo una alta incidencia en Sandino, y esta predominó en los lugares con mayor densidad poblacional con transmisión familiar y laboral intensa en un corto período de tiempo. Introduction: COVID-19 was declared a pandemic by the World Health Organization on March 11, 2020 due to its complexity and rapid global expansion. Sandino municipality presented its first positive case on October 6, 2020. Objective: To characterize the clinical-epidemiological behavior of COVID-19 in Sandino municipality, in 2020. Methods: A descriptive and cross-sectional research was conducted from March 11 to December 31, 2020. With the information obtained from clinical-epidemiological records, virological results, home and work screening, care for acute respiratory infections in clinics and polyclinics, and the daily reports of the Municipal Health Directorate, a statistical analysis was carried out whose information was inserted in an Excel database. Results: The incidence rate was 528.23 per 100 000 population; no deaths. Most of the cases occurred in the city of Sandino. There was a higher frequency of patients in the ages from 41 and 60, and the male sex; while 55.73 percent were asymptomatic at diagnosis. The most common symptoms were cough, fever and nasal congestion. Between one and 13 cases per day were diagnosed during the 63 days that there was positivity. Family transmission was 68.23 percent and occupational transmission was 50.52 percent of cases. Conclusions: COVID-19 had a high incidence in Sandino municipality, and this predominated in places with higher population density with intense family and work transmission in a short period of time; also, in the male sex, the age group in working ages, and in the asymptomatic patients(AU)

Este estudo teve como objetivo analisar o número de casos e óbitos por Covid-19 em relação a gênero e idade, quantidade total de vacinas aplicadas e relação entre a aplicação das vacinas e número de mortes, de janeiro a novembro de 2021, na cidade e região de Viçosa, Minas Gerais. Os dados foram fornecidos pelo Setor de Vigilância Epidemiológica do município e pelos boletins diários da prefeitura. Em média, houve mais de mil casos de infectados/mês de janeiro a julho, a maior parte adultos do gênero feminino. A mortalidade também foi maior entre as mulheres. Foi registrada queda do número de casos e de mortes coincidente com a aplicação da primeira dose de vacina em pessoas sem comorbidades e com início de aplicação da segunda dose nos grupos prioritários. Tal achado indica a importância da imunização coletiva no controle da pandemia.


This study aimed to analyze the number of cases and deaths from Covid-19 in relation to gender and age, the total vaccines applied, and the relationship between the vaccine application and the number of deaths between January and November 2021, in the municipality and region of Viçosa, state of Minas Gerais. Data were provided by the Epidemiological Surveillance Sector of the municipality of Viçosa and by the Daily Bulletins, published by the city hall. On average, there were more than 1,000 cases per month from January to July, mostly female adults. Mortality was also higher for females. There was a drop in the number of cases and deaths, coincident with the application of the first dose of vaccine in people without comorbidities and the initial period of application of the second dose in priority groups, indicating the importance of collective immunization to control the pandemic.

Complex formation processes of tetrasulfosubstituted cobalt(II) phthalocyanine with ORF3a accessory protein of SARS-CoV-2 coronavirus were studied. The interaction of ORF3a protein with SARS-CoV-2 virus with tetrasulfosubstituted cobalt(II) phthalocyanine affords a stable complex in which metallophthalocyanine exists in the monomeric form. The complex formation induces slight changes in the secondary structure of the protein by increasing the fraction of disordered fragments of the polypeptide chain. The photoirradiation of the complex of ORF3a protein of SARS-CoV-2 virus with tetrasulfosubstituted cobalt(II) phthalocyanine leads to the photooxidation of amino acid residues of the protein.

BACKGROUND: The hemostasis system has been extensively investigated in patients in the acute phase of coronavirus disease 2019 (COVID-19). In contrast, the post-COVID syndrome is a poorly known entity, and there is a lack of information on the mechanisms underlying the hemostasis abnormalities in the post-COVID period. AIM: To analyze the potential changes in the parameters of the hemostasis system in the post- COVID period in the plasma of donors with different titers of anti-SARS-CoV-2 IgG. METHODS: The plasma from 160 donors who had recovered from COVID infection was used in the study. Based on the results of the Abbott SARS-CoV-2 IgG serological assay, all donors were divided into several groups: 5 ± 3 (n = 20); 55 ± 5 (n = 20); 65 ± 5 (n = 20); 75 ± 5 (n = 20); 85 ± 5 (n = 20); 95 ± 5 (n = 20); 125 ± 5 (n = 20); 175 ± 5 (n = 20) Index (S/C). A total of 20 healthy individuals without anti-SARS-CoV-2 IgG constituted the control group. Key laboratory parameters, such as fibrinogen concentrations, soluble fibrin monomer complex (SFMCs), and Ddimer, were investigated. In addition, the qualitative composition of the fraction of SFMCs was analyzed. RESULTS: The slight increase in the concentration of fibrinogen, SFMCs, and D-dimers in some donor groups have been found, which could cause the development of hemostasis disorders. In the fraction of SFMCs, the increase in the number of protein fragments with a molecular weight of less than 250 kDa and an increase in the level of proteins with a molecular weight of more than 270 kDa was revealed. CONCLUSION: The obtained results indicated the relationship between the changes in the parameters of the hemostasis system and the titers of anti-SARS-CoV-2 IgG in donors in the post-COVID period. It can be assumed that donors with higher titers of anti-SARS-CoV-2 IgG (>55 ± 5 Index (S/C)) are more prone to hemostasis abnormalities in the post-COVID period since a pronounced imbalance in the levels of SFMCs and D-dimer characterizes them. The appearance of protein fragments of different molecular weights in the fraction of SFMC points to uncontrolled activation of biochemical processes involving molecules of fibrinogenic origin. Additional studies are required to elucidate the role of anti-SARS-CoV-2 IgG in the post-COVID period.

Introduction: controlling the worldwide pandemic, coronavirus disease (COVID-19), could be impossible due to the hesitancy about the available vaccines and the difficulty to implement strict restrictions. Little information is available about herd immunity in the highly vulnerable region of North East Africa, Egypt. Objectives: to assess the seroprevalence of SARS-CoV-2 during the pandemic in one of the highly vulnerable populations in Egypt, Fayoum district of Fayoum Governorate. Additionally, to assess the predictive value of symptoms and other associated risk factors towards a positive COVID-19 test. Methods: in this cross-sectional community-based pilot study, immunoglobulin G (IgG) antibodies that are specific for the SARS-CoV-2 spike (S1-RBD) protein were tested during the period from February 2021 to July 2021. Results: out of 155 participants, 60.6% were SARS-CoV-2 seropositive. Out of symptomatic and asymptomatic individuals, 76.5% and 56.2% were seropositive, respectively. Surprisingly, only one individual had received the COVID-19 vaccine. Previous history of COVID-19; such as symptoms and gender are statistically significant predictors of high seroconversion independent of age, comorbidities, and level of education. Conclusion: this study which disclosed unexpectedly high SARS-CoV-2 seroconversion among the Egyptians, might provide a clear insight into COVID-19 transmission patterns and state of immunity. Further study with a larger sample size on a large scale is required to represent the whole local population.

In order to test the antiviral activity, a series of usnic acid derivatives were synthesized, including new, previously undescribed compounds. The activity of the derivatives against three strains of SARS-CoV-2 virus was studied. To understand the mechanism of antiviral action, the inhibitory activity of the main protease of SARS-CoV-2 virus was studied using the developed model as well as the antiviral activity against the pseudoviral system with glycoprotein S of SARS-CoV-2 virus on its surface. It was shown that usnic acid exhibits activity against three strains of SARS-CoV-2 virus: Wuhan, Delta, and Omicron. Compounds 10 and 13 also showed high activity against the three strains. The performed biological studies and molecular modeling allowed us to assume that the derivatives of usnic acid bind in the N-terminal domain of the surface glycoprotein S at the binding site of the hemoglobin decay metabolite.

BACKGROUND: Slovenia ranks amongst the countries with the highest recorded alcohol consumption. The mortality rate attributed to alcohol-related causes of death in Slovenia also exceeds the EU average. The aim of our research was to confirm the changes in alcohol consumption in Slovenia during the SARS-CoV-2 virus pandemic on a representative sample and to identify vulnerable groups at higher risk of increasing alcohol consumption. METHODS: Two consecutive data collections of the National Survey on the Impact of the Pandemic on Life, each in different epidemiological situations, were conducted. A structured questionnaire was used to monitor the number of alcoholic beverages consumed during the pandemic, compared to the time before the pandemic. RESULTS: The majority of the population did not change the number of alcoholic beverages consumed, and among those with changes, there were significantly more of those who drank less than those who drank more. Among respondents who drank a greater number of alcoholic beverages, statistically significantly higher proportions were found in younger age groups, people with post-secondary vocational education or higher, and people with a higher probability of mental health problems. CONCLUSIONS: During the pandemic crisis, we need to pay special attention to vulnerable groups that are at higher risk of increasing alcohol use.

Desde o início da pandemia, no final de 2019, até a retomada das atividades presencias de atendimento ao público nos cursos da área de saúde em Minas Gerais, especificamente nos cursos de odontologia; foram elaboradas notas técnicas, recomendações e manuais de biossegurança. Documentos esses que serviram como referencial para a padronização de protocolos de atendimento, incluindo o uso de EPI's mais eficientes no controle da transmissibilidade da Covid-19. A pesquisa de revisão foi realizada na base de dados PubMed/MEDLINE e Scielo, World Health Organization, Ministério da Saúde, Associação Brasileira de Ensino Odontológico, Conselho Federal de Odontologia, Conselhos Regionais de Odontologia dos Estados de São Paulo e Minas Gerais e instituições de ensino superior. Priorizando os conteúdos publicados entre os meses de janeiro a setembro de 2020. Período correspondente ao início da pandemia até o retorno das atividades de atendimento nos cursos de saúde em Minas Gerais. Buscando os documentos que nortearam esse retorno e que justificaram a adoção do uso desses EPI's pelos graduandos.


Since the beginning of the pandemic, at the end of 2019, until the resumption of face-to-face activities to serve the public in health care courses in Minas Gerais, specifically in dentistry courses; technical notes, recommendations and biosafety manuals were prepared. These documents served as a reference for the standardization of care protocols, including the use of more efficient personal protection equipment to control Covid-19's transmissibility. The review search was carried out in the PubMed/MEDLINE and Scielo databases, World Health Organization, Ministry of Health of Brazil, Brazilian Association of Dental Education, Federal Council of Dentistry, Regional Councils of Dentistry of the States of São Paulo and Minas Gerais and college. Prioritizing the contents published between January and September 2020. Period corresponding to the beginning of the pandemic until the return of care activities in health courses in Minas Gerais. Seeking the documents that guided this return and that justified the adoption of the use of this personal protective equipment by college students.

The speed and efficiency of student adaptation largely determines the success of the educational process. This is especially true for universityfreshmen, as the older adolescent organism, with its specific neurophysiological characteristics, is very sensitive to changes in the environment. Thesituation with SARS-CoV-2 virus (COVID-19) pandemic has added to the problem of adaptation. The present study aims to examine the influence ofthe COVID-19 pandemic on the first-year students’ adaptation to university studies in Russia. The study involves 692 freshmen with an average ageof 18.7, majority of women (80.6%), from three Russian universities: Kazan National Research Technological University, Kazan Federal University andKazan Aviation University. The results show that during the COVID-19 period statistically significant changes occurred in almost all the adaptationcomponents: physiological adaptation (p < .0001), socio-psychological adaptation (p < .05), academic adaptation (p < .05). There was a decreasein the level of physiological adaptation, while socio-psychological adaptation and academic adaptation increased. Comparison by gender duringthe COVID-19 period shows differences between adaptation processes of men and women. Comparison of foreign students with Russian citizensduring the COVID-19 period demonstrates that foreign students stand out in socio-cultural adaptation (4.94 - foreigners, 4.64 - citizens of Russia, p< .05), physiological adaptation (5.36 - foreigners, 4.36 - citizens of Russia, p < .00001) and academic adaptation (5.28 - foreigners, 4.99 - citizensof Russia, p < .05). La velocidad y eficiencia de la adaptación de los estudiantes determina en gran medida el éxito del proceso educativo. Esto es especialmente cierto para losestudiantes de primer año de universidad, ya que el organismo de los adolescentes mayores, con sus características neurofisiológicas específicas,es muy sensible a los cambios en el entorno. A las habituales dificultades de adaptación se ha sumado la problemática generada por la pandemiadel virus SARS-CoV-2 (COVID-19). El presente estudio tiene como objetivo examinar la influencia de la pandemia COVID-19 en la adaptación delos estudiantes de primer año a los estudios universitarios en Rusia. El estudio involucra a 692 estudiantes de primer año con una edad promediode 18,7 años, la mayoría mujeres (80,6%), de tres universidades rusas: Universidad Tecnológica de Investigación Nacional de Kazán, UniversidadFederal de Kazán y Universidad de Aviación de Kazán. Los resultados demuestran que durante el período COVID-19 ocurrieron cambios estadísticamente significativos en casi todos los componentes de la adaptación: adaptación fisiológica (p <.0001), adaptación socio-psicológica (p <.05),adaptación académica (p <.05). La comparación por género durante el período COVID-19 muestra diferencias entre los procesos de adaptación dehombres y mujeres. La comparación de estudiantes extranjeros con ciudadanos rusos durante el período COVID-19 demuestra que los estudiantesextranjeros destacan en adaptación sociocultural (4.94 - extranjeros, 4.64 - ciudadanos de Rusia, p <.05), adaptación fisiológica (5.36 - extranjeros,4.36 - ciudadanos de Rusia, p <.00001) y adaptación académica (5,28 - extranjeros, 4,99 - ciudadanos de Rusia, p <.05). Coronavirus disease 2019 represents the pandemic of the 21st century that has negatively affected the lives of the whole of humanity. For many months, the only weapons to fight against this infection were protective masks and social isolation. During recent months, fear of the virus has led people to avoid crowded environments and events, and to reduce medical checks, limiting them only to emergencies. Outpatient clinics, doctors' offices, and all closed-in environments were required to limit the patients' access. Nowadays, the presence of specific protocols around the world, and the extended vaccination campaign, have allowed a reduction of many restrictions. Unfortunately, the virus is still widespread, and dental practice and dental treatments suffer the consequences. Dental therapies in general, and in particular orthodontics, are not considered lifesaving. Due to this, orthodontists, in this historical time, must find solutions for motivating patients to start or continue therapies, while providing a safe way for them to do so. There are orthodontists who have developed, during this period, different ways to help them in treating and communicating with patients. Aim: The aim of this study is to assess the influence of the pandemic on the choice to start orthodontic treatment, oral health care, and the importance placed on the appearance of dental occlusions. Materials and Methods: This study is a survey analysis of 159 people, which was posted in Facebook groups of adult orthodontic patients. The timestamps and answers of responses were analyzed to avoid duplicated or interrupted questionnaires. Conclusions: This study found that the current health emergency has not reduced the demand for orthodontic care, while some patients' behaviors are changing in relation to oral hygiene and the importance that they attribute to dental health. It seems that dentists' availability plays a key role in this period of sanitary emergency.

Covid 19 infection can result in various opportunistic infections. Altered immune response, associated comorbid conditions like diabetes, prolonged steroid and broad spectrum antibiotics usage with ICU interventions lead to increased chance of such superadded infections in covid 19 patients. Here we report a case of 31 year old male with covid 19 infection who had new onset Diabetes Mellitus, developed severe acute fulminant Mucormycosis during the treatment for covid 19 infection. He presented with severe headache, nasal obstruction and discharge, bilateral ophthalmoplegia and blindness with Cavernous Sinus Thrombosis. Patient underwent aggressive surgical debridement with frontal craniectomy, maxillectomy and right orbital evisceration and left endoscopic orbital decompression and judicious efforts to revert back the immunocompromised status with high dose of Liposomal amphotericin. Post operatively patient developed right temporal lobe abscess which was drained through Endonasal Endoscopic Trans Cavernous drainage of Temporal Lobe Abscess. Patient is in 2 months follow up, with bilateral blindness and tolerating oral feeds with improved left eye ophthalmoplegia. It is imperative to note that the rising trend of mucormycosis is there in Covid 19 infections and it should be dealt with high index of suspicion in high risk patients and early aggressive treatment can save the patient as the survival rate is not high in such infections.

The year 2020 will be remembered as the year in which a pandemic caused by the SARS-CoV-2 virus precipitated a major disruption in the functioning of contemporary societies. A global event with peculiar regional consequences. It is in this context that we will discuss the ethical aspects of the actions under the responsibility of public officials, namely the ones on national Brazilian relevance for the confrontation of COVID-19. The analysis of the pandemic's effects in Brazil should be based not only on the events triggered at the current moment, whose transience is still an insufficiently known factor, but also on the social, political, and historically economic determinants that heavily interfere in the present events, as well as in the post epidemic future, highlighting the possible scenarios that the political normative, governmental, social, and economic choices underway point to. The tragedy of our time once again presents us with a challenge that is not new, the challenge of a new order, a global order of survival and, therefore, necessarily, a new ethic, an ethic of a global and profound responsibility. This path can only be treated with wisdom and compassion through a model of responsible governance.


El 2020 será recordado como el año en el cual una pandemia causada por el virus SARS-CoV-2 precipitó una mayor disrupción en el funcionamiento de las sociedades contemporáneas. Se trata de un evento global con peculiares consecuencias regionales. Es en este contexto que discutiremos los aspectos éticos de las acciones bajo la responsabilidad de los agentes oficiales, particularmente aquellos de relevancia nacional para el tratamiento del COVID-19 en Brasil. El análisis de los efectos de la pandemia en el país debiera estar basado no solo en los eventos desencadenados en el momento presente, cuya provisionalidad constituye un factor aún insuficientemente conocido, sino también en los determinantes sociales, políticos y económicos que históricamente han tenido una alta injerencia en los eventos actuales, así como en el futuro pos-pandémico, resaltando los escenarios posibles a los que apuntan las políticas normativas, gubernamentales y sociales y las alternativas económicas en curso. La tragedia de nuestro tiempo nos confronta una vez más con un desafío que no es nuevo, el reto de un nuevo orden, un orden global de supervivencia y, por lo tanto, necesariamente, con una nueva ética, una ética de una profunda responsabilidad global. Este camino solo puede ser abordado con sabiduría y compasión a través de un modelo de gobernanza responsable.


O ano de 2020 será lembrado como o ano no qual uma pandemia causada pelo vírus sars-CoV-2 provocou uma grande ruptura no funcionamento das sociedades contemporâneas. Um evento global com consequências regionais peculiares. É nesse contexto que discutimos os aspectos éticos das ações que estavam sob responsabilidade de agentes públicos, a saber, os agentes de relevância nacional no enfrentamento da covid-19 no Brasil. A análise dos efeitos da pandemia no Brasil deve se basear não somente nos eventos desencadeados no momento - cuja transição ainda é um fator insuficientemente conhecido -, mas também nos determinantes sociais, políticos e historicamente econômicos que interferem significativamente nos eventos atuais, bem como no futuro pós-epidêmico, com destaque para os possíveis cenários que as escolhas políticas normativas, governamentais, sociais e econômicas em andamento apontam. A tragédia do nosso tempo nos apresenta novamente um desafio que não é inédito: o desafio de uma nova ordem, uma ordem global de sobrevivência e, portanto, necessariamente, uma nova ética, uma ética de uma responsabilidade global e profunda. Esse caminho só pode ser tratado com sabedoria e compaixão mediante um modelo de governança responsável.

La Covid-19 en niños puede ocasionar cuadros graves al igual que en adultos, en nuestro medio no se conoce la caracterización de virus respiratorios de relevancia causantes de infección respiratoria aguda grave (IRAG) desde el inicio de la pandemia por Covid-19. Objetivo: evaluar el comportamiento epidemiológico de SARS CoV-2, VSR y FLU como responsables de IRAG en población internada en el Hospital del Niño Manuel Ascencio Villarroel en Cochabamba. Métodos: se incluyó a 41 pacientes de hasta 5 años de edad, internados durante junio 2021 a junio 2022. Se empleó la reacción en cadena de la polimerasa (RT-qPCR) para la detección de SARS-CoV-2, VSR y FLU tipo A y B. Resultados: en el 47,6 % de los pacientes se detectó VSR, en el 42,9 % SARS-CoV-2 y en 9,5 % se detectó coinfección entre SARS CoV-2 y VSR, no se presentó casos de infección por FLU. Se reportó principalmente con el 76,2% fiebre y 61,9 % tos. El 14,3% de los pacientes ingresó a terapia intensiva, 2 pacientes fallecieron, uno presentó coinfección viral SARS CoV-2/ VSR y el otro infección viral simple por SARS CoV-2. Conclusiones: tras el desconfinamiento después del inicio de la pandemia por Covid-19 se encontró como agentes causantes de IRAG a VSR y SARS CoV-2 con una frecuencia de circulación similar. Las manifestaciones respiratorias son más frecuentes, mostrando en la mayoría estados estables y recuperación favorable. Es necesario una constante vigilancia epidemiológica ante la experiencia vivida por la pandemia por Covid-19.


Covid-19 in children can cause serious cases just like in adults. In our environment, the characterization of respiratory viruses that cause severe acute respiratory infection (SARI) is not known since the beginning of the Covid-19 pandemic. Objective: to evaluate the epidemiological behavior of SARS CoV-2, RSV and FLU as the cause of SARI in patients admitted to the Manuel Ascencio Villarroel Children's Hospital in Cochabamba. Methods: 41 patients up to 5 years of age admitted from June 2021 to June 2022 were included. Polymerase chain reaction (RT-qPCR) was used to detect SARS-CoV-2, RSV and FLU type A and B. Results: RSV was detected in 47.6% of patients, SARS-CoV-2 in 42.9%, and coinfection between SARS CoV-2 and RSV in 9.5%. There were no cases of FLU infection. Fever and cough were reported mainly in 76.2% and 61.9% respectively. 14.3% of patients were admitted to intensive care, two patients died, one with a viral coinfection SARS CoV-2/RSV and the other with simple viral infection by SARS CoV-2. Conclusions: after the easing of restrictions following the start of the Covid-19 pandemic, RSV and SARS CoV-2 were found to be the agents causing SARI with a similar frequency of circulation. Respiratory manifestations are more frequent, showing mostly stable states and favorable recovery in most cases. Constant epidemiological surveillance is necessary given the experience of the Covid-19 pandemic.

Introducción: El 11 de marzo de 2020 se confirmaron los primeros casos de la enfermedad en Cuba. Objeto: Examinar una complicación de la infección por SARS-CoV-2 infrecuente en edad pediátrica y su tratamiento. Presentación del caso: Lactante de 2 meses de edad producto de un parto distócico por cesárea a las 41,5 semanas, procedente de la provincia de Guantánamo. Por condiciones maternas permaneció con su madre en el hospital durante 45 días. Egresa y a los 15 días en casa, consultan por fiebre de 38 ℃ durante tres días consecutivos. Se constata frecuencia cardiaca muy elevada y estado eutérmico. El test de antígeno y las pruebas de reacción en cadena de la polimerasa con transcriptasa inversa y PCR para SARS-CoV-2 fueron positivas, por lo que se ingresa en terapia intensiva pediátrica con impresión diagnóstica de taquicardia supraventricular que se define finalmente como flutter auricular. El tratamiento aplicado controló la arritmia cardiaca y contribuyó a su favorable evolución posterior. Conclusiones: Los hallazgos del examen físico y los datos en la anamnesis permiten realizar el diagnóstico del paciente en su etapa temprana. La fisiopatología de las complicaciones en niños está en la regulación local de la expresión de los receptores ACE2, aumento de la producción de catecolaminas y una mayor incidencia de toxicidad por medicamentos que producen lesión cardíaca aguda y arritmia, síntomas que pueden estar asociados con disfunción del sistema respiratorio y cambios hematológicos o no en la edad pediátrica.


Introduction: On March 11, 2020, the first cases of COVID-19 were confirmed in Cuba. Purpose: To examine a complication of SARS-CoV-2 infection uncommon in pediatric ages and its treatment. Case presentation: A 2-month-old infant from Guantánamo province was delivered by cesarean section at 41.5 weeks. Due to maternal conditions, the newborn remained with the mother in the hospital for 45 days. Then the newborn was discharged and after 15 days at home was evaluated due to fever of 38 ° C during three consecutive days. A very high heart rate and euthermic state were observed. The antigen test and reverse transcriptase polymerase chain reaction and PCR tests for SARS-CoV-2 were positive, so the newborn was admitted to the pediatric intensive care unit with diagnostic impression of supraventricular tachycardia that is finally defined as atrial flutter. The treatment applied controlled the cardiac arrhythmia and contributed to its subsequent favorable evolution. Conclusions: The findings of the physical examination and the data in the anamnesis allow the diagnosis of the patient in its early stage. The pathophysiology of complications in children is in the local regulation of ACE2 receptor expression, increased catecholamine production and an increased incidence of toxicity by drugs that produce acute cardiac injury and arrhythmia, symptoms that may be associated with dysfunction of the respiratory system and hematological changes or not in pediatric age.

Introducción: La enfermedad por el coronavirus 2019 (COVID-19) es causada por el síndrome respiratorio agudo severo coronavirus-2 (SARS-CoV-2). Además de la invasión tisular directa, el SARS-CoV-2 también puede provocar una respuesta inmune exagerada del huésped, que con frecuencia conduce a una tormenta de citocinas, que contribuye, significativamente, a la disfunción multiorgánica. Las enfermedades autoinmunes se caracterizan por un estado proinflamatorio, aterosclerosis acelerada e incremento de la incidencia de complicaciones vasculares por lo que en estos pacientes existe un alto riesgo de complicaciones ante la presencia del SARS-CoV-2. La alteración en la regulación del sistema inmunológico, el aumento de la demanda metabólica y la actividad procoagulante probablemente explican parte del mayor riesgo de resultados adversos en las personas con enfermedad aterosclerótica relacionada con la COVID-19. Objetivo: Actualizar los conocimientos del personal de salud en el manejo de estas enfermedades. Método: Se realizó una búsqueda de artículos en PubMed, SciELO y Google Scholar con estrategia de vocabulario controlado, de diciembre 2019 a noviembre 2020. Se seleccionaron un total de 50 artículos. Desarrollo: La elevada producción de autoanticuerpos ya sea secundaria a la enfermedad reumática o por una infección intercurrente, puede estar implicada en el desarrollo de la disfunción endotelial, la progresión y la ruptura de la placa aterosclerótica. Conclusiones: Se ha demostrado que estas enfermedades crónicas autoinmunitarias se asocian a una elevada morbimortalidad cardiovascular por lo que el médico de estos tiempos tiene que ser capaz de reconocer estos pacientes(AU)


Introduction: Coronavirus disease 2019 (Covid-19) is caused by severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2). In addition to direct tissue invasion, SARS-CoV-2 can also elicit an exaggerated host immune response, often leading to a cytokine storm, significantly contributing to multi-organ dysfunction. Autoimmune diseases are characterized by a pro-inflammatory state, accelerated atherosclerosis and an increased incidence of vascular complications, which is why there is a high risk of complications in the presence of SARS-CoV-2 in these patients. Altered immune system regulation, increased metabolic demand, and procoagulant activity likely explain part of the increased risk of adverse outcomes in people with Covid-19-related atherosclerotic disease. Objective: To update the knowledge of health personnel in the management of these diseases. Method: We reviewed articles in PubMed, SciELO and Google Scholar with a controlled vocabulary strategy, from December 2019 to November 2020. We selected a total of 50 articles. Findings: The high production of autoantibodies, whether secondary to rheumatic disease or due to intercurrent infection, may be involved in the development of endothelial dysfunction, the progression and rupture of the atherosclerotic plaque. Discussion: It has been shown that these autoimmune chronic diseases are associated with high cardiovascular morbidity and mortality, so the doctor of these times must be able to recognize these patients(AU)

Resumen: La pandemia por SARS-COV-2 ha generado muchos interrogantes sobre el comportamiento de enfermedades crónicas. Si bien se conoce que los pacientes con comorbilidades importantes tienen un mayor riesgo de complicaciones, no se conoce el comportamiento de enfermedades, como la enfermedad pulmonar obstructiva crónica (EPOC), en el período de pandemia y cuarentena por COVID-19. En este artículo, se realizó una revisión de la literatura sobre los principales aspectos teóricos para entender las posibles interacciones entre la infección por SARS-COV-2 y pacientes con patología respiratoria como la EPOC. Se revisaron los principales aspectos de las infecciones virales y su fisiopatología en la exacerbación de la enfermedad, sus mecanismos inflamatorios, y las condiciones que pueden aumentar el riesgo de infección y gravedad durante la pandemia por COVID-19. Además, se exploraron los aspectos relacionados a la cuarentena y su posible influencia sobre la exacerbación de la enfermedad. Finalmente, se exploraron los aspectos económicos relacionados con la exacerbación y su manejo asociado a la pandemia.


Abstract: The SARS-COV-2 pandemic has raised many questions about the behavior of chronic diseases. Although it is known that patients with significant comorbidities have a higher risk of complications, the behavior of diseases, such as chronic obstructive pulmonary disease (COPD), during the period of the COVID-19 pandemic and quarantine is not known. In this article, a literature review was carried out on the main theoretical aspects to understand the possible interactions between SARS-COV-2 infection and patients with respiratory pathology such as COPD. The main aspects of viral infections and their pathophysiology in the exacerbation of the disease, their inflammatory mechanisms, and the conditions that can increase the risk of infection and severity during the COVID-19 pandemic were reviewed. In addition, aspects related to quarantine and its possible influence on the exacerbation of the disease were explored. Finally, the economic aspects related to the exacerbation and its management associated with the pandemic were explored.


Resumo: A pandemia de SARS-COV-2 tem gerado muitos questionamentos sobre o comportamento das doenças crônicas. Embora se saiba que pacientes com comorbidades significativas apresentam risco aumentado de complicações, o comportamento de doenças como a doença pulmonar obstrutiva crónica (DPOC) não é conhecido. No período de pandemia e quarentena por COVID-19. Neste artigo, foi realizada urna revisão da literatura sobre os principais aspectos teóricos para compreender as possíveis interações entre a infecção por SARS-COV-2 e pacientes com patologia respiratória, como DPOC. Foram revisados os principais aspectos das infecções virais e sua fisiopatologia na exacerbação da doença, seus mecanismos inflamatórios e as condições que podem aumentar o risco de infecção e gravidade durante a pandemia COVID-19. Além disso, foram explorados aspectos relacionados à quarentena e sua possível influencia na exacerbação da doença. Por fim, foram explorados os aspectos económicos relacionados à exacerbação e sua gestão associada à pandemia.

Severe acute respiratory syndrome coronavirus (SARS-CoV-2) main protease (Mpro) inhibitors are considered as potential treatments for coronavirus disease 2019, and dietary polyphenols show promise in SARS-CoV-2 Mpro inhibition based on in silico studies. In the present study, we utilize a combination of biochemical-, surface plasmon resonance-, and docking-based assays to evaluate the inhibition and binding affinities of a series of tannins and their gut microbial metabolites on SARS-CoV-2 Mpro. The tested compounds (2-50 µM) were hydrolyzable tannins, including ellagitannins (punicalagin and ellagic acid) and gallotannins (tannic acid, pentagalloyl glucose, ginnalin A, and gallic acid), and their gut microbial metabolites, urolithins and pyrogallol, respectively. They inhibited SARS-CoV-2 Mpro (by 6.6-100.0% at 50 µM) and bound directly to the Mpro protein (with dissociation constants from 1.1 × 10-6 to 5.3 × 10-5 M). This study sheds light on the inhibitory effects of tannins and their metabolites on SARS-CoV-2 Mpro.

RESUMO

The virus severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) causes the disease coronavirus disease 2019 (COVID-19). The cumulative number of cases reported globally is now nearly 197 million and the number of cumulative deaths is 4.2 million (26 July to 1 August 2021). Currently we are focusing primarily on keeping a safe distance from others, washing our hands, and wearing masks, and the question of the effects of diet and diet-dependent risk factors remains outside the center of attention. Nevertheless, numerous studies indicate that diet can play an important role in the course of COVID-19. In this paper, based on select scientific reports, we discuss the structure and replication cycle of SARS-CoV-2, risk factors, dietary standards for sick patients, and the roles of the microbiome and dietary components supporting the immune system in preventing COVID-19.

RESUMEN Han trascurrido dieciocho meses desde que se declaró como pandemia la infección COVID-19 ocasionada por el SARS-CoV-2. No existe aún tratamiento contra el virus, salvo el sintomático y recuperativo. El rápido reconocimiento del nuevo coronavirus permitió la elaboración de vacunas que están siendo usadas de emergencia y han permitido disminuir los casos graves, hospitalizaciones, uso de unidades de cuidados intensivos, ventilación mecánica y muertes. Pero aún quedan incógnitas sobre las modificaciones genéticas y variantes que el virus utiliza para una mejor invasión y adaptación a las defensas del ser humano, así como sobre sus consecuencias inmediatas y a largo plazo en el hombre, la mujer y su niño. Parece distante alcanzar la inmunización de rebaño, porque desconocemos la inmunidad proporcionada por la infección y por las vacunas, así como su duración, y por la importante parte de la población que no desea vacunarse. Además, existe insuficiente capacidad de oferta de vacunas, accesibilidad limitada a los servicios de salud, situación económica desfavorable, incremento de la pobreza y sus consecuencias, disminución temporal de la expectativa de vida y otras complicaciones secundarias a la infección por el virus. La presente revisión es un recuento breve de lo avanzado recientemente en el conocimiento y manejo del COVID-19, en momentos en que el Perú celebra el Bicentenario de la Independencia del país del dominio español, un 28 de julio de 1821, y que encuentra a los peruanos en medio de una pandemia agresiva, elusiva y mortal, y con aspectos humanos, poblacionales, económicos y políticos a resolver.


ABSTRACT Eighteen months have passed since the COVID-19 infection caused by SARS-CoV-2 was declared a pandemic. There is still no treatment for the virus other than symptomatic and recuperative. The rapid recognition of the novel coronavirus allowed the development of vaccines that are being used on an emergency basis and have led to a decrease in severe cases, hospitalizations, use of intensive care units, mechanical ventilation and deaths. But there are still unknowns about the genetic modifications and variants that the virus uses for better invasion and adaptation to human defenses, as well as about its immediate and long-term consequences in men, women and their children. Herd immunization seems distant to achieve, because we do not know the immunity provided by the infection and vaccines, as well as its duration, and because a large part of the population that does not want to be vaccinated. In addition, there is insufficient vaccine supply capacity, limited accessibility to health services, unfavorable economic situation, increased poverty and its consequences, temporary decrease in life expectancy and other complications secondary to infection by the virus. The present review is a brief account of recent advances in the knowledge and management of COVID-19, at a time when Peru is celebrating the Bicentennial of the country's Independence from Spanish rule, on July 28, 1821, and when Peruvians find themselves in the midst of an aggressive, elusive and deadly pandemic, with human, population, economic and political issues to be resolved.
